Middle School Athletics Participation Requirements

Middle School Consent Form

To participate in Franklin Academy's Middle School Athletics Program, all students must complete and submit the following required documents before attending any tryout:

• EL 2 Middle School Pre-Participation Physical Evaluation (Physical Form)

• EL 3 Middle School Consent Form (Parental Permission and Insurance Statement)

• Proof/Copy of current Medical Insurance Card

Completed forms must be submitted to the Athletic Director prior to the first day of tryouts. Students will not be permitted to participate in tryouts until all required paperwork has been received.

Eligibility Requirements

Participation on Franklin Academy's competitive middle school athletic teams is reserved for students who are in good standing with the school.

To be eligible, students must:

• Have all required athletic forms completed and submitted.

• Have no outstanding balances or fees in PowerSchool.

• Maintain a minimum 2.0 GPA in accordance with MSAA eligibility guidelines. Final eligibility will be verified by the Athletic Director after team rosters have been selected.

Athletic Fees

Athletic participation fees will be assessed once team selections have been finalized. All fees must be paid prior to the team's first scheduled competition.
